Your benefits will be reduced if you receive care from a practitioner who is not a Preferred Provider for this plan.

You can look up all providers in a specific area (by zip code, city or county) or you can look up a specific provider by name.

Step 1 - You need to specify zip code, city or county & distance in miles.

Step 2 - You need to specify the type of provider you are looking for (primary care physicians, specialist physicians, hospitals, mental health providers, Urgent Care, radiology center, Vision One providers, etc.). Choose the type of provider you want to find.

Step 3 - Plan - Select "Student Health Plans"

Step 4 - Select your search criteria to focus your search by name, speciality, gender, languages spoken, or find all preferred providers in your geographic area.

Step 5 - Narrow your search to specify the type of physician, gender, languages spoken.
