Graduate & undergraduate students registered for the current term and paying the mandatory fees including the Health Center fee are eligible to purchase this optional insurance plan.
Spouses, domestic partners and unmarried children under the age of 19 are eligible for coverage on the UO insurance plan. The eligible student must be covered by this insurance in order to insure family members. Click here for details.
There are established health insurance enrollment periods at the beginning of each term. Students may submit the enrollment form and premium payment prior to the stated enrollment periods but coverage will not begin before the beginning date of the open enrollment period.
*Contact the Health Insurance Office if you missed an open enrollment period. Students may enroll for health insurance outside the established open enrollment periods if they have involuntarily lost their other health insurance coverage within the past 63 days. A break in health insurance coverage of more than 63 days can effect coverage of pre-existing conditions.
You may pay for the insurance by the year or term. It is your responsibility pay your premium by the enrollment deadline. Health insurance premiums must be paid in full and by the enrollment deadline date. Students who have late financial aid should contact the Health Insurance Office for an “Enrollment Exception.”
This insurance plan is for a limited period of coverage. It is your> responsibility to re-enroll each year or each term to continue your insurance coverage and eligibility to receive benefits. You will not receive a renewal notice.
Students who purchase Fall Term or Winter Term insurance may request billing of the Winter and/or Spring Term premiums on their tuition account. (See Enrollment form) The premium must still be paid in full by the enrollment deadline.
Included in the tuition bill each term is a mandatory student health fee that all registered students must pay. The fee allows registered students to use all University Health Center services.
If a student graduates or takes off a term/semester, Health Center eligibility may be extended for ONE MORE TERM/SEMESTER through payment of the Health Center “stop-out” fee or summer fee.
The health insurance premium and the Health Center fee are two separate charges. Coverage under the University sponsored student health insurance plan does NOT automatically ensure access to the Health Center. It is possible for health insurance coverage to continue longer than a student’s eligibility to use the Health Center.
When a student is no longer eligible to use the Health Center, he/she must obtain their medical care outside the Health Center; regardless of whether the student is still insured on the student health insurance plan.
When you receive medical services at the University Health Center, you will be asked how you want the charges billed. You need to request that charges be billed “Regular” billing.
The Health Insurance Office will bill eligible University Health Center charges directly to the insurance company for you. You do not need to request insurance statements or file claim forms.
If you pay CASH for services or ask for charges to be billed “CONFIDENTIAL”; those charges WILL NOT be billed to the insurance company for you. Contact the Health Insurance Office to make arrangements to bill these charges.
It is the responsibility of the insured person to provide insurance information to medical providers when receiving medical care. Show the insurance ID card whenever receiving medical care. The insurance card lists the policy number and billing address.
If an insured pays cash for any medical services, you will need to provide the Health Insurance Office with receipts.
The insured must pay cash for all prescriptions. Outside pharmacies CANNOT bill the student health insurance for your prescriptions. You will need to provide the Health Insurance Office with the receipts from the pharmacy in order to be reimbursed.
All charges incurred with a medical provider are owed to that provider. It is the responsibility of the insured to make arrangement to pay the deductible and copayment to the medical provider who provided the medical care.
