Vaccine for Children (VFC) Program Fact Sheet

The VFC Program is offered through the Lane County Health Department and provides low cost immunizations for individuals who qualify.

Qualification Requirements:

• Under 19 years of age
• No health insurance coverage of any type
• No coverage by the Oregon Health Plan
• Not a Native American and Native Alaskan

If you meet these requirements you are eligible to receive the HPV vaccine for $12 per dose at the Lane County Health Department immunization clinic. The clinic is held on Wednesday mornings from 8:00 to 11:30 at the Lane County Health Department located at 135 E. 6th Avenue in Eugene. Their phone is 682-4013.
