
Instructions for Telecommunications Order Forms
Form 13 -- Pagers

Department Contact Name: Name of regular or alternate coordinator who is submitting this request. Give two names if the regular coordinator may be out of the office during the work schedule.
- Contact Number: The phone number(s) of the above individual(s). This may include extensions, pagers, off-campus numbers, etc.
- User Name: Individual who will be using the Cellular Phone or Pager ordered.
- Name of the Department.
- Index: Index to be charged for equipment and recurring charges.
- Activity Code: as required by your accountant.
Pager Information:
- Model Desired: Specify which model you require for your User.
- Accessories needed: specify any accessories your User will require in addition to the cellular phone.
Pagers will be mailed to your
department from the Telecommunications Department located at 1244 Walnut
Street or you can make arrangements to come in and pick pagers up.
