Form 12a -- Cellular Phone Service
Changes

Department Contact Name: Name of regular or alternate coordinator who is submitting this request. Give two names if the regular coordinator may be out of the office during the work schedule.
- Contact Number: The phone number(s) of the above individual(s). This may include extensions, pagers, off-campus numbers, etc.
- User Name: Individual who will be using the Cellular Phone or Pager ordered.
- Name of the Department.
- Index: Index to be charged for equipment and recurring charges.
- Activity Code: as required by your accountant.
Cellular Phone Information:
- Index and Billing changes
- Rate Plan Changes
- Name changes
- Cancellation of phone services
Cellular phones are to be picked up at the Telecommunications Department located at 1244 Walnut Street.
