Instructions for Telecommunications Order Forms

Form 1b -- Telecommunications Service Estimate.

The Telecommunications Service Estimate form is to be used to obtain an estimate for the cost of work to be completed by Telecommunications Services for you, work that could be expensive to your department. An example would be for Telecom to install a new telephone jack to be placed in a room that previously has no service.

  1. There is a $25.00 charge for an estimate, which will be applied to cover the Service Order Change ($25.00) if the work is approved and completed. A 10% addition is added into the estimate to cover any unforeseeable charges, but if the actual work is less than the estimate, your department will be charged the lower amount. If the costs are above the estimate, your department will be charged only for the estimate.
  2. Once completed by Telecommunications Services, the estimate is good for 90 days.
  3. When work begins on your order, coordinators will be contacted immediately for approval if the technicians find the job to be much higher than originally estimated.
  4. Fill out the top portion of the form above the bold line. Also attach the applicable forms 1a through 8 to use in the event that the estimate is approved.
    • Building or Address where the work is to be completed.
    • Applicable rental and/or installation index/activity codes.
    • Name of regular or alternate coordinator who is submitting this request. Give two names if the regular coordinator may be out of the office during the work schedule.
    • The phone number(s) of the above individual(s), which includes extensions, pagers, cellular #, etc.
    • Name of the Department.
    • Attached forms 2a through 8 has needed to identify work requested.
  5. Telecommunications Services will try to have the estimate back to you within two weeks, usually sooner, and will not proceed with the work until you approve the estimate by signing the bottom of the form. Once you receive the estimate, please sign this form either approved or disapproved, to indicate whether or not you accept the terms of the estimate, and returning it to this office.
