Survey of Beliefs and Opinions - PARTICIPANT CONSENT FORM 

Principal Investigators: Gerard Saucier & Tarik Bel-Bahar, University of Oregon Psychology Department

This is a study of people’s beliefs. It will take you about 10 to 30 minutes for each of 2 portions of this survey. You will be asked to respond to statements asking you about your beliefs (such as “I am opposed to having a king or queen rule a country” or ” Physical laws cannot explain some mental phenomena”). You will be responding on 5 point scale (1-Strongly Disagree - to 5 – Strongly Agree). Additionally, there will be statements such as “I see beauty that other people pass by without noticing” and words like “faith” and “liberty” that you will responding to on the same scale. You will also be asked questions about your relationship style such as "It's easy for me to be affectionate with my partner" and "How close do you feel to your partner". Although your tasks are fairly simple, it may require some effort to maintain alertness over the many questions that are presented.

CONFIDENTIALITY: Participant names will not be recorded for this study. Your responses can only be identified by a participant number or code. Any data related to your participating in this study will be held in the strictest confidentiality.

VOLUNTARY PARTICIPATION : Your participation in this study is completely voluntary. If you don't wish to participate, or decide to stop at any time, there will be no penalty or loss of benefits which you are otherwise owed. If you decide to participate, you are free to withdraw your consent and discontinue participation at any time without penalty.

RISKS AND BENEFITS : You will receive a summary report in exchange for your participation. There are no other expected benefits to you from participating in this study, although you may gain some insight into the nature and quality of your personal experience. We anticipate that you will experience no harm from participating in this study. Nevertheless please be warned that thinking about certain topics may remind you of negative information. If you feel uncomfortable because of this reason or distressed at any point in the experiment, you may stop participating immediately with no penalty. Please feel free to email questions to the experimenters at any time. As an online participant you can save the text of this page by saving or bookmarking this web page. If you provide your email address later on in the study we will send a debriefing form that explains our research in greater detail.

This study has been approved by the University of Oregon Institutional Review Board.
