Domestic Partnership Registry (Lane County)

*    Couples must be the same sex and at least 18 years old, and at least one partner must be an Oregon resident.

*    Forms are available online at www.uoregon.gov/DHS/ph/chs/index.shtml and www.co.lane.or.us/clerk.  Forms are also available at Lane County Deeds and Records, Public Service Building, 125 E Eighth Ave, Eugene, OR from 8 a.m. to 4:45 p.m.

*   Completed forms must be taken to a notary public prior to being signed by domestic partners.   Once completed and notarized, the forms may be taken or mailed with the appropriate fees to Lane County Deeds and Records, Public Service Building, 125 E Eighth Ave, Eugene, OR 97401.

*   Fees for the domestic partnership registry total $60.  Each couple will receive a certificate and one copy of the form; additional certified copies cost $4 each.
