Measles / Mumps Immunization
The University of Oregon requires proof of measles (Rubeola) and mumps immunity of all entering college students. Without this information a hold will be placed on the registration the term after a student enters the university.
You must meet one of the following five guidelines to assure that you are in compliance with the requirement:
- Measles and Mumps Vaccines: All entering college students born after 1956 are required to show proof of receiving two doses of measles, mumps, or MMR vaccine on or after their first birthday. A Measles and Mumps booster (or MMR booster) is required for students who received one dose. Simply fill out the Measles/Mumps Immunization Certificate form, attach a copy of your immunization records, and mail, fax (541-346-2747), or bring to the Health Center. Be sure to include your full name, date of birth, and student ID number on the form. If immunization information is not available or you do not know your immunization status, the necessary vaccine is available at the University Health Center for a fee.
Note 1: Vaccination records with M-R equals Measles-Rubella vaccine. Vaccination records with MMR equals Measles-Mumps-Rubella vaccine.
Note 2: You can view the measles/mumps immunization certificate form on your browser, and print it out on your printer. You can then mail, fax or bring the completed form to the Health Center.
Note 3: Please obtain and attach a copy of your Immunization Record to your immunization record form.
- A Physician documented history of measles and mumps: Have your physician fill out the medical exemption statement (on the Measles/Mumps immunization certificate), and mail, fax or bring to the Health Center.
- Born before 1957: Please notify the immunization coordinator at the Health Center (541) 346-2764.
- Blood (titer) tests which shows immunity to measles and mumps, (results can take 10 days to get back): Have your health care provider complete the medical exemption statement of the Measles/Mumps Immunization Certificate, attach a copy of the titer test results, and return to the Health Center. (Titer tests are available at the Health Center for a fee.)
- Medical and Religious exemptions: Have your physician complete and sign the medical exemption statement on the Measles/Mumps Immunization Certificate and return to the Health Center. Religious exemption forms are available at the Health Center. You will not be permitted to register for classes without documentation of immunity or obtaining a religious or medical exemption. If exposure to a measles or mumps case occurs, you could be excluded from school for 14 days (incubation period) if you do not have documentation of measles (Rubeola) or mumps immunity.
